COURT HOUSE – Registration for half-day preschool and full day kindergarten will be held on Monday, May 2, Tuesday, May 3, Wednesday, May 4, at Middle Township Elementary No. 1, 215 Eldredge Road, CMCH. REGISTRATION WILL BE BY APPOINTMENT ONLY.
No child will be permitted to enter school for the first time in September if they are not pre-registered. Parents must pick up a registration packet in advance at Elementary #1 School’s main office at that time you will be given an appointment for one of the above dates to return the packet. NO APPOINTMENTS WILL BE MADE BY TELEPHONE.
Preschool students must be 4 on or before Oct. 1, 2011 and Kindergarten children must be 5 by that date.
Registration must be made in person by parent or guardian only. Parent/guardian must bring an original birth certificate, proof of immunizations provided by a physician, two proofs of residency (eg. current tax bill, current utility bill, or lease agreement or an affidavit from our administration office if living with a relative or friend) and if applicable legal papers such as custody or restraining orders. It is not necessary to have your child with you on the day of registration.
