Five of the top seven conditions that constitute the most spending on 15-day readmissions are cardiovascular in nature, and heart failure tops the chart with a 12.5% 15-day readmission rate, according to MedPAC.
Cardiac programs designed to provide specialized sub-acute care to cardiac patients are particularly important to reducing hospital readmissions for Congestive Heart Failure (CHF) patients.
Hospital readmission for CHF is extremely common and quite expensive, although it is frequently preventable. As part of our Cardiac Management Program, we help patients with CHF actively manage their disease along with the physician to ensure a successful and sustainable return to their daily routine.
Based on patient-specific evaluations and tests, treatment plans may include:

• Regular monitoring of blood pressure and pulse.
• Formal rehabilitation therapy designed to address your specific needs including a physical therapy developed exercise prescription.

• Strategies to promote increased activities.
• Medications and medication management to help the heart work better and manage symptoms.
• Education for self-care for a successful discharge.
