Topic: Understanding the History, Implementation, and Impact of POLST throughout the Health Care System
Program Purpose: This program gives an overview of the POLST paradigm. It will discuss the history as well as current trends of POLST paradigm throughout the states. It will provide an overview of the process of POLST and implementation both in healthcare settings and in private homes. The program also focuses on the role that physicians play in the POLST process. Specific strategies will be shared with the audience to improve communication about end of life care.
Audience: RNs, LPNs, SW, CCMC and other healthcare professionals

The following are the objectives of this program:
– Understand the history leading up to creation of POLST paradigm; how and why it came to be and understand what it is; state to state variances
– Understand differences between Advanced Directives and POLST
– Understand the impact of POLST in different health care settings
– Provide specific strategies to improve communication and conversations between patients/families and their healthcare providers
– Demonstrate completion and implementation of POLST form from conversation to policy
