COURT HOUSE – The New Jersey Academy of Family Physicians (NJAFP) recently recognized and awarded Cape Regional Physicians Associates for its innovative work on patient-centered care. The 2015 Patient-Centered Innovation Award was presented at NJAFP’s Advanced Topics in Healthcare Delivery symposium in Atlantic City, NJ on Thursday, June 11, 2015. Cari Miller, MSM, PCMH CCE, NJAFP’s Director of Advocacy and Project Operations, said, “This is the second year we offered this award, and we are pleased that Cape Regional Physicians Associates has demonstrated a commitment to offering innovative approaches to provide patient-centered care.”
Cape Regional Physicians Associates, with ten family medicine and specialty practices located throughout Cape May County, implemented a program that was designed to assist patients and their caregivers through the transition from hospital discharge back to their medical home. Through this effort, the practice was able to impact the post hospital discharge care, through assisting in scheduling the recommended office follow up appointments, assessing the patient’s physical and psychological status post-hospitalization, and educating and reinforcing the discharge plan including medication use, diet and activity.

All of the primary care practices that received NJAFP’s Patient-Centered Innovation Award, including Cape Regional Physicians Associates, have been recognized as Patient-Centered Medical Home (PCMH). A PCMH is a physician office that meets specific standards of care, including a practice in which healthcare professionals work as a team to provide patients with care that is individually determined and meets their specific needs. Healthcare team members strive to know their patients, as well as their preferences regarding care. The team also coordinates patient care with other healthcare professionals in the community. As such, the practice becomes patients’ “homes” for preventive, chronic, and ambulatory care. Under this approach to healthcare, patients are important members of the team. They openly communicate with their healthcare professionals and let the team know when they need more direction and explanation about their medical care.
